Registered Nurses and LPN’s

Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) play essential roles in home health agencies by delivering compassionate, professional care to patients in the comfort of their own homes. RNs typically take the lead in assessing patient conditions, developing individualized care plans, administering medications, and coordinating with physicians and other healthcare providers. LPNs support this care by monitoring vital signs, providing wound care, assisting with daily activities, and ensuring that care plans are followed accurately. Together, RNs and LPNs help patients manage chronic conditions, recover from illness or surgery, and maintain their highest level of independence and quality of life outside of a traditional clinical setting.

Registered Nurses and LPN’s Typically Do the Following:

  • Assess patients’ conditions

  • Record patients’ medical histories and symptoms

  • Observe patients and record the observations

  • Administer patients’ medicines

  • Provide nursing care for treatments, wounds and other clinical services

  • Set up plans for patients’ care or contribute information to existing plans

  • Consult and collaborate with doctors and other healthcare professionals

  • Operate and monitor medical equipment

  • Help perform tests and report results

  • Teach patients and their families how to manage illnesses or injuries

  • Educate patient and caregivers on home self-care

Registered Nurses consult and communicate updates to the patient’s  physicians and other healthcare specialists. Our Registered Nurses also oversee Licensed Practical Nurses, Certified Nursing Assistants, and Home Health Aides.
